	CJIS-011 Applicant Fingerprint Card Note: Only CJIS-011 Applicant Fingerprint Cards that are incremented by 10 may be ordered.
	FBI Applicant Fingerprint Card Note: Only FBI Applicant Fingerprint Cards that are incremented by 10 may be ordered.
	CJIS-012 FBI Only Form Note: Only CJIS-012 FBI Only Forms that are incremented by 10 may be ordered.
	180 Day Request Forms Note: Only 180 Day Request Forms that are incremented by 10 may be ordered.

Delivery of mailed fingerprint cards can be expected in approximately 10 business days. CJIS-CR will mail up to 200 fingerprint cards to the requesting agency. If the agency order supercedes 200 fingerprint cards, your agency representative may arrange to pick up the fingerprint cards at CJIS-CR (6776 Reisterstown Road, Suite 205, Baltimore, MD 21215). A two business day notice is required for coordinating such requests. Pick up must occur between 9am to 4pm, Monday through Friday.
